About Sullivan Case Management

An organization built around steady relationships and practical support.

Our mission

We identified critical gaps in continuity of care: health insurance timelines and requirements can interrupt support before an individual’s needs are fully met; communication can become fragmented during transitions between levels of care and the return home after treatment; and the essential human connection of consistent, peer-to-peer mentorship is often missing. Sullivan Case Management exists to help close these gaps through coordinated communication, trusted relationships, and practical support that extends into everyday life.

Founder

Andrew Sullivan

Andrew Sullivan is the founder of Sullivan Case Management. He brings nine years of experience working within mental health and addiction treatment, giving him a strong understanding of how individuals and families navigate care across different stages of support. A Certified Recovery Specialist through the International Recovery Institute, a NAADAC and CCAPP education provider, Andrew has built extensive relationships with clinicians, treatment providers, and community-based professionals. His experience informs the standards, collaborative approach, and practical support provided by the Sullivan Case Management team.

Andrew does this work because he understands how difficult it can be to translate treatment into meaningful change in daily life. After struggling with mental health and substance use beginning at age 14, he spent two years moving in and out of treatment programs. Although those programs appeared to offer the right support, he experienced firsthand how gaps in continuity, communication, and practical guidance can emerge during treatment and transitions. Andrew ultimately found a path to lasting change and today has nearly a decade of continuous sobriety.

He has built a fulfilling life centered on fitness, travel, and community, and brings that same emphasis on purpose and connection to Sullivan Case Management. His years working within the behavioral health treatment system provide valuable insight into navigating care, building effective professional teams, and carrying treatment goals into everyday life. He founded Sullivan Case Management to provide the consistent, relationship-based support he believes is essential to meaningful, lasting change.
